Diabetes mellitus is an important risk factor for stroke. Stroke prevention can not be achieved by blood glucose control alone but can be by simultaneous control of blood pressure, cholesterol and visceral obesity. For the management of acute ischemic stroke in diabetic patients, special attention should be paid to the treatment of hyper- and hypo-glycemia, occurrence of hyperosmolar non-ketotic coma, and use of contrast material. For the secondary prevention of stroke, strict management of risk factors as well as antithrombotic therapy is required
